Is your feature request related to a problem? Please describe.
When someone changes something and adds a comment at the end, it is often related to his change or to further investigate it would be useful to know, the user changed something. As a OSM contributor it's very hard to detect this situation as the note contains the same, as there would have been just a "normal" note from OM.
(The UI shows clearly, the text field at the end is to "create a note", but often it is misunderstood as a "commit/change message or description")

Describe the ideal solution
When changing and adding a comment in the created note, there should be the changeset link or id listed too.

Describe alternatives you have considered
Maybe it would reduce the note "cluttering" with commit messages, when having a extra field for this and writing it directly into the changeset comment. (But it wouldn't solve the issue, when someone wants to create a note, which will or could relate to his change)
